7th consecutive deficiency-free survey

State Dept. of Public Health's annual survey of Maple Crest Assisted Living

Maple Crest Assisted Living, Bluffton, is deficiency-free as a result of its 2020 annual State Department of Public Health survey, which took place on Jan. 16.

It's among of the highest confirmations of quality service for an assisted living community. 

This is an especially outstanding accomplishment because this was Maple Crest’s 7th consecutive deficiency-free survey.

Daren Lee, director of Maple Crest, said, “This achievement was made possible by the daily commitment and continued dedication of a caring team.”  

The survey is conducted annually by the Ohio Department of Health and includes a checklist of areas and categories to examine.

Some of the categories include meeting with residents, reviewing medical records, reviewing the kitchen and resident dining areas and observing the environment and safety factors of the home.

Ultimately, the survey evaluates the overall quality of life and quality of care of the facility to ensure it is operating by the Department of Health’s standards.
